    Home Companion Solutions (HCS) and Elder Care Solutions (ECS) provide a continuum of care supporting older adults and families across Chicagoland. From hands-on companion care to RN-led care management and advocacy, we are deeply committed to helping clients age safely and confidently.

    We are growing—and looking for someone who wants to grow with us. Our office is conveniently located in the West Loop (close to the Green line and Union Station) with plans to move to Oak Park in the near future. The position will require local travel, in office work and work from home opportunities.


    Position Overview
    This is a dynamic, relationship-driven role that combines client intake, community outreach, and operational support.
    You’ll be the first point of contact for new clients, a key relationship builder in the community, and a trusted support within our office operations.

    What You’ll Do
    Client Intake & Care Coordination
    • Respond to new client and referral inquiries
    • Conduct intake calls and assess client needs
    • Coordinate start-of-care logistics and caregiver matching
    • Ensure a seamless onboarding experience for clients and families

    Community Outreach & Referral Growth
    • Build and maintain relationships with referral partners (assisted living, hospitals, case managers, bank estate planners and trust manager, etc.)
    • Develop new referral sources through outreach and in-person visits
    • Representing the company at community events and meetings
    • Manage ongoing communication with partners (email, phone, in-person)
    • Support marketing initiatives including social media and outreach campaigns

    Operations & Administrative Support
    • Utilize agency software (WellSky preferred) for documentation and tracking
    • Support payroll and invoicing processes as needed
    • Track employee credentials and compliance requirements
    • Coordinate employee and client satisfaction surveys
    • Assist with new hire orientation, annual employee training, evaluations, and file audits

    What We’re Looking For
    • Strong communication and relationship-building skills
    • Highly organized and detail-oriented
    • Comfortable balancing multiple responsibilities
    • Proactive, self-starter mindset
    • Experience in healthcare, home care, or senior services preferred
    • Experience with WellSky or similar software is a plus

    Compensation & Structure:
    • Base salary: $65,000
    • Quarterly bonus: Based on both HCS and ECS’s growth, specifically tied to revenue growth compared to the prior year (on an annualized basis), with a percentage of that growth shared based on overall performance.  The bonus would be 2% of combined (HCS &ECS) quarterly revenue growth 
    • Paid time off (10 days on an accrual basis) and holidays
    • Mileage reimbursement for community outreach
    • 401(k) with employer match after one year
